Evolving structure of the early church

I think I posted this in the wrong place at first, sorry. I am continuing to go thru the entire 26 series of lectures from Yale University on New Testament History and Literature. Another thread here brought up the idea of God as like the male head of a family.
How did that concept come about? Apparently, for church structure, it actually did not start out that way, but began as a democratic model. Here is the lecture about it:
