PAR Authorization Form - Am I Missing Something?

Just looking at the new (not sure exactly when it was implemented, but it is newer than the first one that came out) PAR Authorization Form and I cannot see anywhere on the form that the giver signs to authorize a pre-authorized debit under Option 1 - Pre-Authorized Debit. There is a place for signature under Option 2: Visa or Mastercard, but this is different from the section where a person attaches a VOID cheque. Am I missing something? Does someone else see something that I dont' see? Any treasurers or church secretaries that work with these forms?

I don't see a signature line. Perhaps by attaching a void cheque & completing all the other information a signature is not required?
I can see that one is needed for credit card authorization - as signature is required for that validation.
Will be interesting to see any other replies.

I wondered that too, carolla, but on the information for PAR it says that "1. Sign-up Date and Signature" are required.
I'm interested in what others have found also. Tomorrow I will phone the office in Toronto and ask them.

OK - here is the scoop in case anyone else is curious.
I had to phone the woman in the PAR office on some other matters so I just asked her about the signature space on the new form. She said it was a "glich" - I took that to mean "inadvertently missed", but that you could have the new contributor sign where it says "Signature" for the Credit Card section, and just ignore the rest of the section on the credit card space.
A nice lady - very friendly and was easy to talk to

Yes, if they know about it, why haven't they fixed it? It's surely just a matter of having the original corrected and re-pdf'd for the link.
